Primary and Secondary School Excursion Booking Form

I will carefully read the Excursion Information, Risk Management and Emergency Response Plan prior to my excursion.

I understand that my students are to be supervised at all times and I will ensure my supervisory team understand this also.

I understand that we are required to make our way around the park in small groups (K-3 = 5 chn per group) or (4-6 = 10 chn per group) and I will ensure my supervisory team understand this also.

Any additional parent-helpers, that the school will be paying for, will be charged at the normal admission rate, and are required to enter with the school.

Any additional parent-helpers, that the school will not be paying for, will enter the park through the public entry.

I will ensure that our bus driver is notified that our bus cannot be left outside the CWP entrance.

I understand that there are no water fountains in CWP and will therefore ensure my students have adequate drinking water.

I will ensure that we have our meal breaks within our designated picnic area.

I understand that the CWP Souvenir Shop is out-of-bounds to school groups.

I will ensure that my students and my supervisory team understand the suggested method of animal-feeding and animal-touching as explained in.
